Please read the Whistleblower Protection Policy carefully before making a disclosure, since only eligible whistleblowers can make disclosures that are eligible for protection under the Corporations Act.
You can make a disclosure anonymously if you wish. To remain anonymous, please include your disclosure in the ‘Description’ box in the form below (attaching any relevant documentation) but don’t provide any other details. If you wish, you can also provide an appropriate email address that permits us to communicate with you but which cannot identify you. This will help us to keep you informed about your disclosure and any investigation – otherwise, we may be unable to do so. Notifications that a form has been submitted are provided to Healthdirect Australia’s CEO (as an Eligible Recipient) and its Whistleblower Protection Officer.
