In accordance with Healthdirect Australia’s Whistleblower Protection Policy, this form allows eligible whistleblowers to submit whistleblower disclosures to a Healthdirect officer eligible to receive those disclosures. Please read the Healthdirect Whistleblower Protection Policy carefully before making a disclosure to ensure you understand what a whistleblower disclosure under the Corporations Act involves, as only eligible whistleblowers making disclosures that meet the requirements of that Act are eligible for protection under that Act.
You can still use this form to make other disclosures of misconduct or an improper state of affairs relating to Healthdirect or its contractors, and Healthdirect will consider them.
You can make a disclosure anonymously if you wish. To remain anonymous, please include your disclosure in the ‘Description’ box in the form below (attaching any relevant documentation) but don’t provide any other details. If you wish, you can also provide an appropriate email address that permits us to communicate with you but which cannot identify you. This will help us to keep you informed about your disclosure and any investigation – otherwise, we may be unable to do so. Notifications that a form has been submitted are provided to Healthdirect Australia’s CEO (as an Eligible Recipient) and its Whistleblower Protection Officer.
